| Summary / Zusammenfassung | The general goal of the Swiss IBD cohort (SIBDC) is to provide the Swiss and international scientific, public health and medical community with a disease-oriented prospective, partly population-based, cohort of IBD patients. This cohort is designed for optimal integration of multidisciplinary collaborations, by harvesting information about epidemiological, environmental, immunological, genetic, clinical, and psycho-social factors as well as outcome of disease and economical information on medical and social resource utilisation. The unique features of the SIBDC is the set-up of its collaborative network, that include 1) a broad base in private and public GI practices and centres, 2) a large potential number of patients with good geographical stability, 3) a prospective data collection, 4) a follow-up system through an existing, anonymous internet-based channel, and 5) its integration with newly established treatment algorithms based on state-of-the-art methodology to define appropriateness of care in Crohn’s disease. In addition, the project is oriented towards international collaborations. In addition to general information such as incidence, prevalence and impact of disease for Crohn’s disease, Ulcerative Colitis and Indeterminate Colitis in Switzerland, the SIBDC will provide prospective, observational, patient-centred data collection, assembling and coupling epidemiological, immunological and genetic information with longitudinal data on treatment, disease and patient outcome, including its economical and psycho-social aspects. The concomitant assemblage of patient biological sample collections (serum, immune cells and gut biopsies) will permit the use of state-of-the-art immunological and genomics tools on patient groups with well characterized phenotypes of disease. The database will be coupled with information on appropriateness of care to evaluate outcome based on appropriateness of therapy, proper use of medication, as well as over- and under-use of therapy. Economic assessment will focus on health care utilisation, private vs. social insurance coverage of costs of care, sick leave allowance, disability allowance. Finally the project will federate a network of physicians and thus create a platform for multi-centred treatment protocols, a particularly important issue at a time when numerous novel therapies are developed against these diseases. The cohort itself may be used for outcomes research projects which could fuel useful information about effectiveness of medical interventions in usual practice. Within the general goal of the project, the scientific committee of the SIBDCS has defined "nested projects" dedicated to focussed aspects of IBD research, one of which is entitled "Pharmacogenetics of Drug Transporters and Nuclear Receptors in Inflammatory Bowel Disease" and is being carried out at the Division of Clinical Pharmacology and Toxicology, University Hospital Zurich.
